New Patient Visits
To expedite your first visit, we recommend that you read and fill out our New Patient Packet, which includes the following information.

Notice of Privacy Practices
The Healthcare Insurance Portability and Accountability Act requires that we safeguard all of your protected health information, with certain exceptions as provided by law. Please take a moment to read our Notice of Privacy Practices.

Financial and Billing Policy
Dr. Tapadiya's office accepts Medicare and most private insurance PPO plans, as well as many employer-based health plans. We are not a provider for any HMO plans. For private insurance plans, we will bill your insurance if we are a participating provider or an out-of-network provider for your plan. However, any uncovered amounts, co-pays, and co-insurance are the patient's responsibility. We strive to work with patients to provide any documentation needed for claims handling. Musculoskeletal and orthopedic care is often complicated with multiple avenues of treatment, and we are happy to discuss and explain any portion of your bill to you at any time. Unless prior arrangements have been made, payment is due in the office at the time service is provided. We accept cash, checks, and credit/debit card payments. It is the patient's responsibility to keep any changes to address or insurance information updated with our office. A complete copy of our financial policy can be found here.

Medical Records Release and Copying Requests
We are happy to release your medical records to yourself or a third party provided that we have your signed permission to do so. In addition, we are happy to copy records or imaging studies for a nominal fee as allowed by state law to cover toner, paper and labor costs. Please print out, review and sign our records release and duplication request form. Records are typically sent well within the 15-day turnaround period allowable by state law.

Disability, Handicapped Placard, and Work Excuse Forms
We are happy to fill out disability and handicapped placard forms, or if you require work excuse forms. We regret that due to the significant chart review and time required to fill out these forms, we must charge a nominal fee as noted here. We do not charge a fee for school/physical education excuse forms.

Prescription Refills
Per our prescription refill policy, we only refill prescriptions from Monday-Friday from 9 AM to 5 PM. No refills are given on weekends. This applies to pain medications as well as non-narcotic medications.
